This privacy policy relates to the personal information collected by Sanis Health Inc. (“Sanis”, “we”, “us” or “our”).
Please read this policy to understand how we handle the personal information that you provide to us. We may make changes to this policy from time to time. Any changes we make will become effective when we post a modified version of the policy on this webpage.
The Sanis website may provide links to other websites. This privacy policy only applies to Sanis and its website and does not apply to any other site that may be linked in any way from this website.
What Type of Personal Information Do We Collect and How?
We define "personal information" as any information that identifies you or could be reasonably associated with you. The personal information we collect may include your name, contact information (email address, phone number), date of birth, sex and country of residence.
We limit the collection of your personal information to what is reasonably required to fulfil the purposes for which it was collected. We collect personal information in a variety of ways, including directly from you, your authorized representative, and/or your healthcare provider by mail, by email or over the telephone, when you experience an adverse event while using a Sanis product. An "adverse event" is any untoward medical occurrence in a patient-administered medicinal product and which does not necessarily have to have a causal relationship with this treatment. An adverse event can therefore be any unfavorable and unintended sign (for example, an abnormal laboratory finding), symptom, or disease temporally associated with the use of a medicinal product, whether or not considered related to this medicinal product.
We may also collect certain types of information electronically when you interact with our website, www.sanis.com, through the use of cookies. Cookies are small text files that are saved on your computer when you visit a website so that information can be saved between visits.
How Do We Use and Share Personal Information?
We use your personal information to determine the severity of an adverse event experienced by you while using a Sanis product, to appropriately document the adverse event details in a report, to communicate with you, and otherwise as permitted or required by law.
We may share your personal information with our service providers (companies operating on our behalf) for the purposes described in this policy and in accordance with applicable law. Service providers must undertake that they will keep all personal information confidential, only use it for the limited purposes of providing their services to us and return or destroy all personal information once they have provided the services.
If we determine that a serious adverse event has occurred involving a Sanis product, we are required to report that event to Health Canada. This report may contain your personal information.
We use cookies to help us provide you with a better experience when you visit our website. We may also use Google Analytics, a web analysis service of Google Inc. (“Google”). Google Analytics uses cookies to analyze your use of our website, to create reports about visitor activities for us and to provide further services associated with the use of websites and the Internet. Although this information is collected through your Google ad settings, it is not provided to us in a personally identifiable format. You may delete or disable certain of these technologies at any time via your browser. However, if you do so, you may not be able to use some of the features on our website.
How Do We Manage Consent?
By submitting your personal information to us in connection with the use of a Sanis product, you are providing your consent to the collection, use and disclosure of personal information as set out in this policy. In some cases, your consent may be “implied” i.e., your permission is assumed based on your action or inaction at the point of collection, use or disclosure of your personal information.
We will obtain consent when we want to use personal information for a new purpose or for a purpose other than those stated in this policy.
Your consent may be withdrawn at any time, except in limited circumstances, including legal or regulatory requirements, by contacting the Loblaw Privacy Office at the contact information set out below. Your decision to withhold certain personal information, or to withdraw consent, may limit the services we are able to offer to you.
How Do We Protect and Store Personal Information?
We take the security of your personal information very seriously and are committed to protecting your privacy by using a combination of administrative, physical and technical safeguards. Access to your personal information is limited to only those authorized personnel that require access in the performance of their duties, to any person granted access by you and to those otherwise authorized by law.
We use all reasonable safeguards, including contractual requirements with our service providers, to protect your personal information wherever it is used or stored. In general, we store, access and use personal information in Canada. Some of our service providers may access, store or use your personal information outside of your province, territory, or Canada in the course of providing their services to us. In that case, the personal information is subject to the law of the jurisdiction in which it is used or stored, including any law permitting or requiring disclosure of the information to the government, government agencies, courts, and law enforcement in that jurisdiction. The countries where our service providers are currently located include Ireland and Germany.
We will store your personal information for as long as necessary to fulfill the purposes for which it was collected, except where otherwise required or permitted by law. When no longer required, your personal information will be securely destroyed or anonymized (so the information no longer identifies you).
How Do I Request Access to or Correct My Personal Information?
Under Canadian privacy law, you have the right to access the personal information we hold about you, subject to any legal restrictions. Upon request, we will provide you with access to your personal information within a reasonable timeframe, in compliance with applicable laws.
